Circumcision - Consent Documentation

I reviewed the risks/benefits/alternatives to circumcision including but not limited to bleeding, infection and injury to penis as well as the inability to guarantee cosmetic result. She is aware that circumcision is not a medically necessary procedure with limited data to suggest any improvement in short or long term health and she still wishes to proceed.
