Cesarean Delivery - Consent Documentation

Counseled on R/B/A to cesarean section, to include risk of bleeding, infection, damage to surrounding anatomical structures (i.e. Bladder, intestines, uterus, fallopian tubes, ovaries, nerves, blood vessels, etc.), blood clots, death, injury to baby, need for hysterectomy. We discussed the possibility of blood transfusion, and she is amenable, if needed. Consents signed today.
