Protocol for PRP Use at Surgery: SOP for Fertility PRP Injection in Gynecologic Surgery

Abstract

A standard operating procedure (SOP) for platelet-rich plasma (PRP) injection into the ovaries, cervix, and uterus during gynecologic surgery. Covers patient selection and contraindications, PRP preparation using 80-120 mL venous blood with dual-spin centrifugation, and injection techniques for ovarian intraovarian injection (4 cc per ovary with calcium activation), uterine/endometrial infusion, and cervical injection. Indications include diminished ovarian reserve, thin/refractory endometrium, Asherman syndrome, poor ovarian response, implantation failure, low cervical mucus, and chronic endometritis.
