Mucus Cycle Score (MCS)

The Mucus Cycle Score (MCS) is a CrMS quantitative classification that scores the six days ending on Peak Day to place a cycle on a five-tier fertility scale: regular, intermediate-regular, intermediate-limited, limited, or dry-cycle.78

Developed by Hilgers as part of the CrMS charting system, the MCS replaces physician-dependent cervical mucus scoring (such as the Insler score) with a standardized method built entirely on external vulvar observations. Each of the six pre-Peak and Peak days receives a score based on mucus quality, and the average determines cycle classification. No speculum exam is required. The method is serially repeatable in any primary care setting.

Classification matters clinically. Limited and dry cycles correlate with infertility and recurrent pregnancy loss, signaling suboptimal follicular estrogen output. Regular cycles correlate with normal ovulatory hormonal production. The MCS gives CrMS charts diagnostic weight beyond fertility timing, supporting targeted restorative hormone evaluation.
