Q-IVF Annual Report 2020: Fertility Treatments in Sweden (Treatment Year 2018)

Q-IVF, National Quality Registry for Assisted Reproduction

Q-IVF, 2020

RRM Academy Synopsis

About 20% of Swedish fresh IVF cycles with own gametes ended in birth

Sweden's national IVF registry reports that about 20% of fresh IVF cycles started in 2018 with the couple's own gametes ended in a live birth, 2 255 of 11 061 cycles at eighteen clinics. The annual report also presents frozen embryo cycles, twin deliveries and complications.

Key Findings

  • Of 11 061 fresh IVF cycles started with the couple's own gametes, 8 066 reached embryo transfer and 2 255 resulted in live births, about 20%.
  • The report names the woman's age as the most important factor in the chance of a live birth with her own oocytes, because oocyte numbers and quality decrease at a higher age.
  • Frozen embryo cycles (7 041 started, 6 787 with embryo transfer) resulted in 2 273 deliveries; the report associates improved results with extended culture to the blastocyst stage.
  • Twins made up 2.54% of deliveries and there were no triplet deliveries; the report attributes the low multiple pregnancy rate to transferring one embryo at a time in most cycles.
  • Complications went unreported in 15% of fresh cycles; those reported included 42 cases of ovarian hyperstimulation syndrome requiring hospitalization, nine cases of bleeding and 11 cases of infection.

Interpretation

The report compiles aggregated registry counts for cycles started in 2018, with almost 100% of Swedish IVF cycles accounted for. The counts describe outcomes after IVF and include no comparison group of couples who did not undergo it. Some started cycles were not intended for embryo transfer, such as freezing eggs or embryos before chemotherapy, and the registry's authors state that live births per started cycle alone can mislead. The authors also note that clinics treat patients of different ages.

RRM Context

IVF bypasses the reproductive system and leaves the cause of infertility in place. Live births per cycle show how often the bypass ends in a birth. The report attributes infertility about equally to the man, the woman, or both partners. It notes that some cases have no explanation and calls them unexplained infertility. Restorative reproductive medicine evaluates both partners to find the cause and regards unexplained infertility as undiagnosed.
